Steps to Complete Your Forms
Please print out the form you need below and fill it out completely, including a "wet" signature (aka print it out and physically sign it).
You can email your form to firstname.lastname@example.org or turn it in at any of our Enrollment Services Centers or fax to (719) 502-2074.

Some forms can be submitted electronically, those who are current students or future students who have already registered for classes may do so via their student e-mail.
***Forms must be completed in blue or black ink. Failure to accurately complete these forms may result in a delay of processing or change of financial aid eligibility. Additional documentation may be requested. Read instructions carefully before completing.
Verification Form 1
Because students sometimes make errors on their application, PPCC has procedures for verifying the reported information. **Your 2018 signed 1040 Tax Forms will be accepted for tax documentation.
Independent Students with a PPCC College assigned email, complete your Verification Worksheet electronically via your myPPCC Portal.
Verification Forms 4 and 5
Verification 4/5 may be completed in person ONLY at the Centennial Campus or Rampart Range Campus on a walk-in basis Monday through Thursday from 8 a.m. - 5 p.m. and Friday from 9 a.m. - 5 p.m. Please bring a valid, government-issued photo ID and a proof of high school completion.
*NOTE - Please do not complete form until you appear in person with a PPCC Financial Aid Advisor.
If you cannot appear in person, you will need to complete the appropriate forms in front of a valid Notary. The notary will assist with completing page one indicating the type of documents submitted for their review. They will notarize the first page, as well as make copies of your ID/ High School Diploma/GED and notate on the copies that they made the copies/date and their initials.

Tax Filing Status
Your 2020-2021 FAFSA application had been identified by the Department of Education for potentially containing conflicting information about either your marital status or your parent's marital status and the Tax Filing Status. You are required to resolve this conflicting information before your financial aid application can be processed.
Independent Students with a PPCC College assigned email, complete your Tax Filing form electronically via your myPPCC Portal.
The income reported on your financial aid application is below the federal estimate of your annual cost of living.
Your 2020-2021 Free Application for federal Student Aid (FAFSA) has been flagged for “Unusual Enrollment History Review” by the U.S. Department of Education because you received Federal Pell Grant funds at multiple education institutions during the review period; 2016-17, 2017-18, 2018-19 and 2019-20.
Independent Students with a PPCC College assigned email, complete your Unusual Enrollment form electronically via your myPPCC Portal.
Applicants who are taking preparatory coursework to enter an undergraduate or graduate degree program may apply, if eligible, to receive a Federal Direct Subsidized Loan for up to $2,625 for one 12-consecutive month period.
